ORDERING INFORMATION FOR THE THERAPIST

Please contact us via the Contact Us link to start the request process. We will email you a form that will require the following information:

• Name of clinic/office
  
• Address of the clinic/office
  
• A phone number that you can be reached
  
• Your name
  
• Patient's name
  

Also included on the form will be the following selections related to the weighted blanket or lap pad:

• Blanket or Lap Pad
  
• Front color choice
  
• Back color choice
  
• Weight (limited to 12 pounds)
  

Once we receive your request, we will respond within 24-48 hours with an estimated delivery time. Each blanket is individually hand-made and prewashed prior to shipment/delivery.
